Inability to create filesystems and the above warnings solved. I was loading lvm-config. Without lvm-config, it worked fine. The issue of hanging at 56% during "Starting up the partitioner. Please wait...." after vg & lv creation is still an open issue. I have not been able to determine why it sometimes hangs and other times it does not. Bottom line: We now have a successful install with lvm, and have successfully resized varlv & /var, a ReiserFS, without umounting the filesystem. I am pleased.
