My guess is that was the issue, though the solution I found at some point was very strange. The output above clearly says "space_cache=v2", but that may not be true. Chrooting into the system and editing fstab so it actually reads "space_cache=v2" for both volumes instead of just "space_cache" did resolve the issue. I can't say why, only that it worked. This issue appeared reproducibly and the solution worked as reproducibly, as I did start over at one point as something else went wrong.
