This seems to also be an issue when a CD is used to kick off a preseed - i.e. Automated install from the boot menu. Depending on the preseed commands used, you'll end up with a cdrom: line in sources.list and a prompt in tty4 to insert a CD. You can't answer that prompt, so the install hangs.
