Make it easy on yourself. When starting the installer process, get to the main menu and set a couple things. First message priority needs to be changed to low option 4. Next log installation and choose hard drive and the mnt directory. After that, do an integrity check on the disk which will go into your log files. Then continue with the installation and if you reproduce the error it will be found in your log files. The debian-boot list will find those log files informative too.
