I don't see the difference between using a trigger and let a cron job
fetch mails. Both solutions require the user to copy mail to a special
folder?
An internal dbmail-imap trigger could detect when a user moves a mail FROM the spam folder. He may move it to Inbox or any other custom folder. You can't detect that with an external script.

With cron scripts there's always the chance that a user deletes the mails before the scheduled time.

The biggest provider I know in germany that offers IMAP has more than
4.000.000 customers.
Hehe, OK. They must have quite a few servers for that.
