/etc/dbmail.conf as defined in dbmail.h iirc. It's location can not (yet) be
controled by specifying ./configure --prefix.

Are you looking at the right dbmail.conf file?

/usr/local/sbin/dbmail-imapd

should do just fine for imap. Check out the init-scripts included in the contrib
directory.

Not that I know of. Pretty trivial to write though for the basic stuff.
