No mta, no mail. You can use any MTA out there together with dbmail, as long as you can somehow get your mta to deliver email to dbmail-smtp or dbmail-lmtpd. All modern mtas have mechanisms similar to postfix's transport.

There's been howto's for using sendmail on the list several times, iirc. Postfix is just the mta used and preferred by most users on the dbmail lists.
