Interesting, I had the same issue and this fix worked for me as well. I have setup dbmail quite a few times and have never seen this issue....... so what is different this time? I am setting up dbmail with postfix 2.0X not 1.X.
I found more info on it in the /etc/postfix/README_FILES/LOCAL_RECIPIENT_README file. George is this true for you as well? AS this is specific to postfix 2.0 might be a good idea to add a note to the INSTALL file.
