I can not address qmail issues; I never got it to work unmodified
before finding Postfix... However, the keys to POP (or IMAP) before
SMTP authentication is getting whatever mail transport agent you're
using to accept entries from a database in determining whether any
given IP connecting to send mail is "local" or not.

In postfix with MySQL, it's as simple as adding a line to the
configuration file, stating that "$mynetworks" includes the pbsp table
that dbmail (optionally!) maintains. If you're using Postgres instead
of MySQL, you need to visit http://www.rhyme.com.au/postfix.html for
patches to let postfix use postgres tables.

Searching http://www.google.com for qmail+postgres+patch shows a
number of sites that have information on getting qmail to talk to both
Postgres and MySQL tables for a variety of features; whether or not
determining authorized relay networks is one of those, I can't say.
