I am using amavis as a contentfilter in postfix and then I wrote a
transport for postfix that runs spamassassin and procmail for each user.
In the .procmailrc script it then calls dbmail-smtp. The transport uses
sudo to allow the dbmail user to execute one of the scripts as root and
then back to the osuser. There is an additional osuser column to the
dbmail.user table. I also created a dbmail-getuser program which can
decode the e-mail destination into the actual dbmail username. This
stuff is not packaged up anywhere, but I can do that if it seems worth
while.
