OK, I may be confused here - why doesn't incoming internet mail arrive
via postfix? Is your inbound MX machine NOT the local machine? (the
rest of this message assumes the local machine is NOT the official
mail exchanger for the domain)

If the mail arrives via internet to another machine, and your machine
is then using POP to pick it up from there using fetchmail for
injection into dbmail on your local machine, that would explain a lot
of problems... Your local setup might be correct, but the upstream
mail server isn't providing the information needed for fetchmail to
properly inject it into dbmail.

Can the upstream machine be configured to relay the mail for your
account(s) to the local machine via SMTP, rather than relying upon
fetchmail to grab it?
