Complete list of out-of-date mirrors

Okay, I decided that someone needs to check for out-dated mirrors and 
that someone is just going to have to be me.

I audited every mirror in mirrors.html.  The results below are from 
hosts that were reachable.  Hosts that were unreachable are not 
reported.  Anything not in the list below was either unreachable or had 
a date stamp on setup.ini of at least 2004/01/11.
