It hasn't been an issue too much lately, but i think it's due to fact
that there needs to be "OK" from valve server to client and to server
before it is allowed to enter. So basically if steam and your server is
out of sync (steam clients report players still playing to steam while
they are not on the server anymore, or something), this might happen.
Though i'm only guessing here.
