A detail worth noting for review: with all four call sites routed through the same `isUpstreamFailure` helper, a 3xx that carries a JSON error body is now reported with that body in the `LlmGatewayException` instead of being handed to the parsers — which is exactly what you want when a misconfigured base URL points at an API portal.
