### What problem does this PR solve?
1. Over `max_scan_key_num` the scan keys no longer cover every value of the
column, so the predicate has to be kept. A BOOLEAN fixed set cannot degrade to
a range, and that branch returned without clearing `exact_value`, so the
caller erased the `IN` predicate and `b IN (FALSE, TRUE)` returned the
NULL rows.
2. A string type has no largest value: `TYPE_MAX` is a one byte `0xff`
sentinel, and real data such as `0xff61` sorts after it. The upper bound is
now a flag rather than that sentinel. `RangeValue` is a `CppType` wrapper
holding it, and
its operators sort the flag above everything, so the comparisons in the
class are unchanged. A column with no real upper bound stays out of the scan
keys and keeps its predicate.
