The road to get a MediaWiki extension stable enough, tested enough, trusted enough and needed enough for it to get rolled out on Wikipedia is long and hard and I wouldn't recommend it unless you have the most cast-iron of use-cases.

If the ISBN support were proposed today, I strongly suspect that it won't make it in today. Of course it's now grandfathered in and removing support seems very, very, unlikely.
