 At least when i was there, major technology projects (basically any requests 
for support that involved more than a single Library IT employee) had to be 
approved by the Project Review Committee through a formal quarterly process. 

My current institution is a much smaller library, where to the best of my 
knowledge, there's no formal policy or review process; requests for our web 
developer's time go through his manager, our Assistant Dean for IT & 
Collections Services.
