To answer the question in your subject line, I'm not sure.  It certainly seems 
like a misuse if not an abuse.  I always do stuff like that in my friend, 
-menuNeedsUpdate.  In -menuNeedsUpdate, you can go so far as to remove all the 
items and rebuild the whole menu from scratch if you want to.

It seems like it is this removal which is not working.  It is odd, but 
documented, that to remove a key equivalent, you must set it to @"".  Setting 
keyEquivalent to nil does not work as expected.
