Use AppleScript (on iPhoto) on the Mac and ALAssetsLibrary on iOS. Both let you have remarkably capable access to the user's photo library, including creating albums and populating them. Exactly how they interact with PhotoStream is something you will need to explore. I have found PhotoStream and other iCloud-based services to be fast and reliable. Use them every day.
