Thanks, Kyle.  That's what we call "brute force" :))  Not very economical, but 
always works!

So I switched on the "Title Bar" checkbox under "Appearance" in the window's 
Attributes.  Problem solved.  Confirmed by a couple cycles of retesting.

I'm calling this a bug in AppKit.  Unless someone offers a better explanation 
I'll file with Apple.

Thank you guys for lending your brains to this.  Probably truncated by brute 
forcing by an hour or two.
