I've noticed a bug in IB toolbar editing, where the default toolbar
configuration does not get saved properly if the first item is a
button or segmented control (maybe other things, but buttons and
segmented controls are the only things I tried).
Try putting a space separator or a vertical line separator as the
first item in the toolbar and see if that solves your problem.
