I made a small test app (available on request) which exhibits the same problem. For the first few minutes Xcode shows App Nap Prevention and Idle Prevention both gray. Then it starts napping and things go awry: timers get delayed by 10 seconds etc. So you were absolutely right with “App Nap or timer coalescing”.
