John,

if you want to be able to reach to the very top of the mast, get yourself a 
harness and an aider (e.g.: 
http://blackdiamondequipment.com/en_CA/big-wall-climbing/alpine-aiders-BD3900500000ALL1.html).
 Or make yourself a pair of stirrups that you can attach to the a halyard (and 
a harness). You can stand up at the top of the mast. Well, at least you can 
have your torso above the top. Makes the work there much easier.

I always say that the sailors should learn the working aloft techniques from 
alpinists and the alpinists should learn the line handling techniques from 
sailors (;-).

I think it would be difficult to change without taking the mast down. I had to 
move halyards last year and did it with the mast up and it was all by feeling 
because I could not get high enough in the bosun's chair.
