The second most common villain is usually congestion at the gateway. Common causes are bit torrent, downloads that consume all inbound bandwidth, virii/malware, bad or inadequate hardware (ie. a cheap or an old router unable to cope with the traffic thrown at it), to name a few. I would recommend getting a handle on your network and ISP first.
Some tools to help you: iperf, hping, mtr, netcat, iftop, iptraf. Personally I have used providers from Montreal and various places in the US without issue. Also, it should be noted that having a gateway in Calgary may not get you what you want. One of my connections is a Rogers portable Internet and when I connect to my computer at home (cable modem from Shaw), the packets actually travel all the way to Ontario and back again even though I am physically in Calgary.
