Making count return a correct long or BigInt value in such cases would be a significant change, given that Java's int is used for the return value for many count() and countFrom() methods sprinkled around Clojure's source code.
I've created a ticket CLJ-1229 and attached a patch that causes count to throw an ArithmeticException if the sequence contains more than Integer/MAX_VALUE elements.
