On a mail server most people would use a milter, e.g. clamav-milter, which is part of the ClamAV package. The use of milters offers many benefits. It enables a mail server to inspect a message during the SMTP conversation, allowing the server (for example) to reject unwanted mail at the earliest possible time, before accepting the message.
