well, yes, install clamd, start clamd and configure the milter to use
the clamd socket you defined or the clamd instance over TCP
did you realize that the milter is nothing else than a client talking
to clamd and plays the glue between the MTA and clamd itself?
You didn't tell us if you indeed have Clamd running - please verify by
running:
clamdscan --reload
