ClamAV can't be relied upon to make up for the shortcomings of your
mail system.  If it is regularly accepting cr@p you need to fix it -
if only because you are encouraging the miscreants, who also pester
the rest of us.

To begin with consider blocking IP address ranges which send cr@p to
you regularly.  All suspicious mail should be REJECTed.
