I've not found any particular advantage. I use J-Chkmail as a Sendmail 
milter for  spam and it invokes clamd for attachments. I've never found 
a reason to change it. This very simple combination has provided me with 
a very good way to provide a clustered MTA environment with load 
sharing, fail-over, urlbl, grey-listing daemon, anti-spam, Bayes, regex, 
behavior, malware scanning via clamd, rrd-tool reporting, and aside from 
the rrd-tool has the bonus of not using or needing Perl.

The only other milter feature that interests me is that found in 
milter-ahead.
