I think this is not a job for clamav, but a job for your MTA.

In Exim, for example, I think it would be sensible to reject the message 
[that might persuade the virus that your domain is not a good target], but 
perhaps to then generate another message to the recipient saying "someone 
<address> sent you a message with subject <subject>, but we rejected it 
because we think it may have contained a virus".
