That behaviour is correct and anticipated.

Before the first database update, the header lists the ClamAV and
clamav-milter versions, but not the database version; after the update,
it lists the ClamAV and database version, but not the clamav-milter
version. Wouldn't it be more consistent if it contained all of these,
in the same format, in either case? Something like:
X-Virus-Scanned: ClamAV 0.90/2600, clamav-milter devel-120207 on
my_server.example.org
