I am using ClamAV 0.88.7 on OpenBSD 4.0 on almost 5 servers ( migrated
from RedHat Linux to openBSD ).
On two servers I faced the problem that frequenlty the mails would
stop coming in. The clamd would be utilizing upto 90% of cpu. Then I
would kill the clamd & mail flow would restart.
While I was unable to find the issue one day I noticed that the /tmp
was full ( /tmp is about 300mb on these servers ) . I had then changed
the /etc/clamd.conf to have tmp dir in /home/tmp ( /home is almost 2Gb
free , chown _clamav._clamav /home/tmp & chmod 1777 /home/tmp ).
Restarted Clamd and since then no problem till now.

[PS: I have also asked earlier that has anyone used Mail::ClamAV on
OpenBSD 4.0, without any response. On Redhat it works like charm. ]
