I resorted to using --external.  The best way I could come up with
notifying clamav-milter of new dbs was to restart the milter.  I suppose
increasing the timeout in sendmail for connecting to the milter would
(only slightly) alleviate any worries about something slipping through
while the milter was unavailable.

Out of curiosity, what is the "best practice" of notifying the milter?
Is simply restarting it the way?  Is there a signal that can be
passed to clamav-milter when freshclam finds new dbs?
