Hello,
Nice to know.
I was able to set the flag I wanted in /etc/sysconfig/clamav-milter
  Now starting sendmail shows maillog...

/usr/sbin/clamav-milter --quiet -N --external --dont-wait

That was easy. Now I need to see if it cleans the virii
and I suspect is should.
