That not a bad idea, you only need another machine running qmail with
clamav in front of qmail servers. This way, you dont need to modify your
current email servers. If your are not sure about to requested changes,
this is the best solution.

This may sound strange, but why not put another box in front of the qmail
server just running as an smtp relay and virus scanner? You don't mention
traffic loads, but if you're not running spamassassin on the bastion box, you
shouldn't need much of a cpu.
