This happened the other day and local mail delivery ground to a complete halt (I think outbound mail delivery did too.)
What OS? What version of ClamAV? What MTA? If sendmail, are you using the milter? If you're using the milter, are you using --external or any other interesting flags?
It happened again at around 7AM this morning, I gave up and rebooted the machine again at 2PM this afternoon.
Rebooted?? Are you on Windows?
Assuming you're using clamav-milter, you might try using clmilter_watch, which can be used to detect failures and restart the milter out of cron. Of course, that doesn't fix your issue, but it will at least allow your machine to fix itself in a timely manner, rather than having to wait for you to fix it.
