does the path to the socket match between sendmail and clamav-milter ? permissions on socket?

The INPUT_MAIL_FILTER should be one line (assuming email wrap)
Delete the "define(.." entry It effectively remove the defininition from the line above. not what you want. (ref: http://www.sendmail.org/m4/adding_mailfilters.html)
recompile sendmail.mc and restart sendmail.
The clamav-milter docs need to be updated.
