Generally, it is purely for extra options and configurability. The likes of Mailscanner, Amavis, etc tend to incorporate the facility to scan for various types of content, or correctness of the mail composition, by calling upon various seperate programmes. Expandability would probably be the simplest way to put it. Also, a milter is Sendmail specific, (as far as I know), whereas external programmes like the above are designed to work with most MTA's.
