I tested 0.81rc1 with zlib 1.2.2.2 on linux and clamd stops respond
after several hours/days to clients (clamav-milter, clamdscan) requests.
There must be STREAM related problem, I think.
Try attached patch. And report - it fix problem or not.
I have limited time resources, so did applied both patches together - your's thrmgr.c.patch
and Tomasz Kojm's comment_out_SESSION_define_in_clamav-milter.c.patch).
The clamd on testing machine runs 48 hours about without problem.
But after upgrade to zlib-1.2.2 on FC3 linux some system utils like zcat started became shaky.
Im going to downgrade to vendor patched zlib 1.2.1 and test it again.
