You've only included log outputs from clamd, not from clamav-milter. Please post the relevant syslog entries from clamav-milter.

The problem appear as the first message arrive and I have in /var/log/maillog a lot of these:
Dec 5 00:50:57 kirk sm-mta[45825]: iB4NovjD045825: Milter (clmilter): error connecting to filter: Connection refused by localhost
Dec 5 00:50:57 kirk sm-mta[45825]: iB4NovjD045825: Milter (clmilter): to error state
and in /var/log/messages:
Dec 5 00:50:57 kirk /kernel: pid 45821 (clamav-milter), uid 999: exited on signal 11
