I like http://www.mailscanner.info as it neatly handles all the email / file 
unpacking requirements before handing an entity to ClamAV for virus 
diagnosis, and it also provides some very useful filename / filetype 
filtering functions of its own (ie: even if an attachment isn't infected with 
a virus, you can block it just because the filename ends in .scr, if you 
want).   It's also highly configurable for sender / recipient email addresses 
& domains, allowing you to create different filtering for different groups of 
users, if you need that sort of thing.
