There shouldn't be any need for (significant) downtime - build the new clam and install. Then restart the clam daemon, which shouldn't take more than a second or two.
If you're not using the daemon, then you don't even need to do that.
That's assuming nothing goes wrong. If that's a production system, you HAVE to plan for downtime. if clamd isn't working, most likely his mta isn't delivering mail.
