I found that setting a higher max-children threshold helped the situation for me - milter processes still hang all the time, but eventually they do die, and as long as one doesn't actually hit max-children, the system as a whole keeps on working. It would be really nice to actually have the bug fixed properly though, rather than resorting to work-rounds like that.
