Assuming that clamscan can pick up Eicar when you scan it standalone (ie: it 
is working and has a functional signature database), I would (a) check the 
permissions on MailScanner's incoming directory, to ensure that the user 
under which ClamAV is running can scan the files, and (b) run MailScanner in 
debug mode (see the end of the MailScanner.conf file for information on this, 
or ask on the MS mailing list) to see more detail about what happens when it 
calls ClamAV.

I certainly run MailScanner + ClamAV with no problems, just following the 
standard install procedures for each product.
