You've lost me there. I didn't bother include the text at the top of the
report that says it's from MailScanner. ClamAV is the only program on
our server that scans email.

It hardly looks like a report from ClamAV. ClamAV doesn't detect
HTML forms because forms aren't viruses.
Most probably you've got some other software that scans email, besides
ClamAV.
