This is not really a ClamAV question since ClamAV doesn't detect spam, and doesn't care which user the email is going to. http://www.mailscanner.info is the sort of thing you want - it has excellent rulesets for doing different things for different users, it integrates with ClamAV (as well as a large number of other antivirus engines) and it can use SpamAssassin to detect spam as well as its own internal checks.
