In the source distro for clamav, there is clamd.sh

find clamav-XXXXX/ -type f -name "clamd.sh" -exec cp /etc/rc.d/init.d/clamd
cd /etc/rc.d/init.d/
checkconfig add -levels blah blah clamd

man checkconfig
