I hate to ask, but do you have "ScanArchive" in your clamav.conf? Anyway, put a file owned by the user that MD runs as ('defang' or whatever) in your MD spool directory and try scanning it with clamdscan. At least that way you can figure it out without having to deal with MD and e-mail in the mix.
