Please, set the proper line lenght in your "Ooopslook" - currently you
write lines which are far toooo long (see above), which makes answering
(helping you!) harder.

On the topic:

I don't use clamav-milter so I don't know what command exactly should be
used for starting. I mean I can't verify your suggested command.
As on creating your own init script: you may use the Debian example
file:  /etc/init.d/skeleton

Or have a look at the bug report #204340 in which the example solution
is described:
