MailScanner (http://www.mailscanner.info) has a list of "Silent Viruses" in response to which it will not send notifications to the (apparent but false) sender. It also does an excellent job of detecting spam, mostly with the aid of SpamAssassin, which I believe you don't get with clamav-milter. Just a suggestion for a way to possibly upgrade your system...
